本研究の目的は、精神科未治療・治療中断者の受療行動を促すための、地域と協働した交渉スキル習得プログラムを開発することである。初年度である2022年度は文献検討を行い、研究の枠組みを検討した。そして、「未治療・治療中断の人の受療行動を促すための交渉」とは、孤立傾向にあり、病感はあるが病識の欠如があり、受療の意向がない患者に対し、受療の必要性を認識している専門職が行うものであると考えた。専門職は、知識不足とスティグマをもつ患者と家族に対し、徹底的に寄り添い、本音と可能性を追究することで交渉の基盤を築き、患者のみならず地域や学校、職場も方向づけ、患者と巧みに押し引きして、患者と家族の自己決定による合意へ導くプロセスであると捉えた。そして当初計画を見直し、2023年度目標の一部である「精神科未治療・治療中断者が地域で受けた支援と、受療するまでのプロセスを明らかにする」ことを2022年度の目標とした。研究組織のネットワークを用い、精神科病院と併設する訪問看護ステーション、精神障害者への訪問看護を標榜する訪問看護ステーション、精神科アウトリーチを実施する自治体や精神科病院や診療所、保健所の施設長に、研究協力候補者の推薦を10名程度依頼することとした。研究協力候補者の選定条件は、①未治療・治療中断の期間があったが、現在までに治療を継続するようになった患者、②未治療・治療中断時の思いや、家族をはじめとする身近な人、専門職などとの関わりについて回想してインタビューに答えられる患者、③インタビューに答えることができることができるほど病状が安定していると専門職である支援者に判断された患者である。現在倫理審査を終え、研究協力候補施設へ研究依頼を行っている。なお、当初予定の「精神科未治療・治療中断者の受療行動を促すための地域との協働による交渉を明らかにする」ことは、2023年度後半に実施予定である。
