介護保険制度の導入が目前に迫っている。本制度では、要介護高齢者の介護上の手のかかり具合、すなわち日常生活自立度をさまざまな方法で評価する。障害性老人の日常生活自立度の判定基準としては、「寝たきり老人の日常生活自立度(J1,J2,A1,A2,B1,B2,C1,C2)」や「痴呆性老人の日常生活自立度(I,IIa,IIb,IIIa,IIIb,IV,M)」が従来から用いられてきた。そこで本研究では、高齢者の課題分析の道具として広く普及している「在宅版高齢者アセスメント表(MDS-HC)」のデータセットからこれらの障害性老人の自立度を推定できるかどうかを検討し、MDS-HCが要介護度判定に利用可能かどうかを検討した。方法としては、障害性老人の各自立度を規定する症状や障害をMDS-HCの対応する項目に置き換え、これらの項目のチェック状況から自立度を推定できるようにコンピュータ・プログラムを組んだ。つぎに実際に高齢者を対象として、上記判定基準による評価とMDS-HCによる評価をそれぞれ行ない、MDS-HCから導き出される自立度と従来の判定基準による自立度の一致率を検討した。その結果、これら二つの自立度の間には高い相関が認められ、MDS-HCにより障害性老人の日常生活自立度が推定可能であることが明らかとなった。

The introduction of the protection system is currently under pressure. This system is intended to protect the elderly and the elderly. The criteria for determining the degree of self-reliance in daily living of the elderly with disabilities include "degree of self-reliance in daily living of the elderly with disabilities (J1,J2,A1,A2,B1,B2,C1,C2)" and "degree of self-reliance in daily living of the elderly with dementia (I,IIa,IIb,IIIb,IV,M)." This study aims to analyze the subject of elderly people with disabilities and to explore the possibility of using MDS-HC to determine the degree of self-reliance of the elderly with disabilities. Methods: To define the individual status of the elderly with disabilities, and to organize MDS-HC responses to symptoms and disorders, and to estimate the degree of independence of the elderly with disabilities. In addition, the evaluation of MDS-HC and the evaluation of MDS-HC are discussed. The results show that there is a high correlation between self-reliance and MDS-HC, and the self-reliance of the disabled elderly is estimated.
