This subproject is one of many research subprojects utilizing the resources provided by a Center grant funded by NIH/NCRR. The subproject and investigator (PI) may have received primary funding from another NIH source, and thus could be represented in other CRISP entries. The overall goal of this project is to determine which girls with premature pubarche are at risk of developing PCOS. Three hypotheses will be tested. Hypothesis 1: The 17-hydroxyprogesterone (17HPROG) response to a human chorionic gonadotropin (hCG) test of ovarian function will be higher after gonadotropin releasing hormone agonist (GnRHag) than alone. Hypothesis 2: The magnitude of the 17HPROG response to hCG will correlate with the number of ovarian follicles on ultrasound, the 17-hydroxypregnenolone (17HPREG) response to ACTH, and indexes of insulin resistance. Hypothesis 3: The highest quartile of testosterone levels will be found in patients homozygous for a candidate activating variant ("G-variant") of the 17¿-hydroxysteroid dehydrogenase type 5 gene. To test these hypotheses, 6-9 year old girls with premature pubarche will undergo glucose tolerance, cosyntropin, gonadotropin releasing hormone agonist (leuprolide), and human chorionic gonadotropin testing and a pelvic ultrasound examination.
