This subproject is one of many research subprojects utilizing the resources provided by a Center grant funded by NIH/NCRR. The subproject and investigator (PI) may have received primary funding from another NIH source, and thus could be represented in other CRISP entries. The institution listed is for the Center, which is not necessarily the institution for the investigator. Changes in diet and lifestyle can improve symptoms of polycystic ovary syndrome (PCOS), including hyperandrogenemia and menstrual disturbances. However, no studies have evaluated the postprandial response to meals of varying diet composition in women with PCOS. The primary objective is to determine if meals of different fat and fiber content will affect plasma testosterone concentrations in women with PCOS. The second objective is to see if meals of different fat and fiber content affect circulating glucose, insulin and gonadotropin (LH and FSH) concentrations. We will recruit women age 19-40 with polycystic ovary syndrome (chronic oligo/anovulation and hyperandrogenemia) who are not taking any confounding medications (hormonal contraceptives, insulin sensitizing agents, antihypertensives, antipsychotics) or are currently pregnant. All subjects will complete an informed consent and medical history and undergo a physical exam, dual energy x-ray absorptiometery (DXA) and transvaginal ultrasound. Subjects will be served the test meal that is to be consumed within 15 minutes. The meal will either consist of: (1) egg and cheese croissant and whole milk or (2) cereal, low-fat milk and an apple or banana, yogurt. After each meal, 3-mL blood samples will be taken every 10 minutes for 6 hours for FSH and LH measures and at 30 minutes and every hour, 20 mL will be taken for glucose, insulin, testosterone and SHBG. Glucose levels will be monitored continuously using the MiniMed Continuous Glucose Monitoring System. Participants will participate in both conditions with test meals being separated by at least seven days.
