Collecting and tracking accurate and reliable information from heart failure patients and patients at risk of heart**failure is a continuous challenge, and a burden on our healthcare system as the process is typically resource**intensive. Currently, during a patient visit information is gathered and entered into an Electronic Medical Record**(EMR), which is sporadic and typically not analyzed. In many cases, this becomes a missed opportunity to provide**the patient with the patient-centered care and custom health required to improve their health outcomes.**Asclepios Cardiology Corporation, located in Ottawa Ontario, is a clinical healthcare practice encompassing**experienced, regulated Ontarian Cardiologists and Nurse Practitioner providing high quality, timely, cardiac**services to Ontario patients. Asclepios is proposing the development of Heart Failure, a mobile app that will**enable patients to record symptoms, input experiences and autonomously input other data using the technology**of heart functions with details such as weight, blood flow, heart rate and descriptions, etc. A mobile app, would**allow for continuous feeds in real live tracking, providing positive patient outcomes such as reduced hospital**admissions, hospitalizations predictions, improved quality of care and preventing the onsite of disease.**Asclepios is looking to collaborate with Lambton Colleges Information Technology & Communication Research**Centre to explore, research and develop the novel mobile app, Heart Failure. The research team, that will include**two research technicians and one research student, will work closely with the Asclepios team to develop the**mobile app using Artificial Intelligence and Machine Learning.
