Portal 724 provides Tele-Health Solutions over a secure Canadian Platform including a software solution for Remote Patient Monitoring (RPM), Personal Health Records (PHR) and wellness predictive software. Additionally, they design and manufacture devices and sensors for use by patients, homes, hospitals, and senior residences. It is imperative for medical professionals to be able to have access to accurate patient information whenever and wherever they need it. Connecting medical devices to an Electronic Medical Record (EMR) system gives an immediate access to accurate patient information which helps improve clinical decision-making and patient safety. In collaboration with Lambton College's Information Technology & Communication Research Centre (ITCRC), Portal 724 plans to develop an interface with the major Canadian EMRs to interchange health data and integrate with existing platforms. The success of this project will bring many benefits to the healthcare infrastructure including increased quality of care (especially after discharge), chronic disease management, evidence-based decision making, sharing patient data within care teams, access across the continuum of care, efficiency, transmission and communication of results, patient empowerment and a significant reduction in healthcare service costs.
