本研究では、東京都をモデル地域として周産期センターの適正配置に関する分析のための基礎データの収集ならびに解析を行った。さらに、地理情報システム(GIS)利用の一貫として健康状態と地域収入格差の関係についての分析も行った。まず、GISソフトウエアに東京都における産婦人科医療機関の地理分布情報を入力し、対象人口と保健サービス供給ポイントの地理的関係についての分析を行った。具体的には、各医療施設から既存の7つの周産期センターへの最短距離をそれぞれ測定し、地域別に各周産期センターが担当すべき医療施設の地理区分図を作成した。GIS分析により現在の周産期センターの分布と最適解との差が確認された。次に、東京都の道路地図と医療施設情報を組合せより正確な医療施設の地理区分図の作成した。さらに、将来的な周産期センターを増設する場合にどの地域に配置すべきかの検討を行った。代表的な医療施設における分娩数、母体搬送数、病床数、医療従事者数、設備などのデータ入手と地域の妊婦や分娩数の将来予測を行い、これらとGISによる解析との統合がこのモデルの将来的課題である。一方、GISを用いた医療システムの解析の一環として地域の収入格差と個人の主観的健康状態の関係について社会疫学的分析も行った。わが国では収入格差が増大しており、欧米における先行研究では2者の間に有意な関係が認められている。しかし、本研究では収入格差は有意ではなく、個人の収入ならびに地域の収入の平均値と健康状態との関係が示唆された。これは、先行研究における地域の収入格差変数が地域の特性や政策の差異を示しており、収入格差との交絡バイアスによって説明できると考えられる。今後、地域単位をさらに絞り込み、GISによる地理情報を加えた上でさらに分析を進める予定である。

This study is based on the analysis of regional and regional distribution patterns in Tokyo. Number of births, number of maternal transfers, number of hospital beds, number of medical workers, number of medical facilities, number of pregnant women, number of deliveries, number of future predictions, number of GIS analyses, number of future problems, etc. The analysis of the relationship between the regional income difference and the health status of the individual is carried out in the field of social epidemiology. The difference between the two countries is increasing, and the relationship between the two countries is increasing This study shows the relationship between income and health status. This is the first study of the differences between the regional income levels and the characteristics of the region and the differences in policies. From now on, geographical information will be added to the analysis, and the future will be determined.
