「高齢者保健福祉推進十か年戦略」でも、ホームヘルプ・サービスは在宅福祉の三本柱のひとつとして位置づけられ、在宅要介護老人の介護を支えるサービスとして期待はますます高まっている。本研究は在宅高齢者の介護を支える家族介護者とホームヘルパーの各々のストレス軽減に関与する諸要因を、ストレス理論に基づく分析枠組や測定尺度を用いて明らかにすることを目的としている。今回はホームヘルパーのストレスに関する調査を実施し、その結果を分析することに重点をおいたが、松岡が1994年に実施した家族介護者のストレスに関する調査結果との比較も試みた。ホームヘルパーに関しては、長野県内のホームヘルパー479名を対象とした調査から得られたデータ(有効回答率78.7%、有効票377)を用いて分析した。調査は市町村社会福祉協議会の協力を得て行われた。ストレッサーおよびストレス反応の測定指標の作成と、その妥当性、信頼性に関して検討を加え、ホームヘルパーのストレス構造と関連要因との関係を検討した。ストレッサーは、ホームヘルパーとして働く中で遭遇する不快な出来事となるものを想定して12項目から成るスケールを作成した。これらの項目はある程度の頻度をもって出現し、スケール内の信頼性係数は.80であった。因子分析の結果「ケ-ルに関わる因子」「上司や同僚に関する因子」「勤務に関する因子」の3因子から構成されていることが明らかになった。ストレスの反応の測定は、12項目からなるスケールを作成し、因子分析によって「抑うつ気分・怒り因子」「不安因子」「不機嫌因子」の3因子が抽出された。ホームヘルパーのストレスの軽減には排除できるストレッサーを取り除くと共に、欠勤時に代わりのヘルパーがケースを訪問する体制を整え、保健医療との連携を図るなど、組織的資源の充実を図ることが有効であることが明らかになった。
