项目摘要

要介護高齢者を介護している家族介護者の介護ストレスや負担感について,その影響要因と介護者のコーピングの様態を解明し,家族介護者のエンパワーメント支援に関するメカニズムを析出することを目的にしている.平成20年度から実施している家族介護者へのインタビュー調査,デイサービスを利用している在宅家族介護者412名を対象として平成21年度に実施した配票調査の結果を基に,本年度は「要介護3」以上の高齢者を在宅で介護しているN市内の全ての介護者4,349名に対して調査依頼をし,同意を得た1,330名から有効回答を得た結果を分析した.対処行動のうち介護者の実行度が高いのは,「あきらめ」「肯定的思考」などの認知的な対処であり,反対に実行度が低いのは「友人・親戚への援助依頼」「介護者同士の励まし合い」「情報収集」などの行動的な対処であった.因子分析の結果,「積極的認知」「同調・回避的認知」「問題解決的行動」3因子が抽出された.「積極的認知」(α=.80)を従属変数とする多元配置分散分析の結果,高年齢者ほど積極的な認知をしていることが示された.対処行動のうち効果的だと評価されたのは「気分転換」「家族の協力」「介護サービス利用」「専門家の援助」であり,評価されなかったのは「友人・親戚への援助依頼」であった.各項目の実行得点と効果得点にはほとんどに相関関係が認められ,特に「同調・回避的認知」は実行していないと効果的ではないと評価される傾向が強かった.また,「問題解決的行動」因子の中でも「気分転換」は実行していない人にも効果的と評価される傾向が認められた.介護ストレスをためない工夫に関する自由記述の分析では251単語が抽出され,5つの対処パターンに識別できた.趣味,外出などの気分転換型が43.0%で最も多く,次いで公的支援型21.0%であった.家族介護者支援Webシステムを開発中であり,本稼働に向けて準備中である。
