项目摘要

包括的家族カンファレンスの基盤作成のために、今年度は主に家族終末期の移行期における看護介入について文献検討を実施した。CHINAL,MEDLINE,web of scienceのデータベースにより探索語は、ACP,Home Health Nursing,(OR Home Care Servies)とし対象文献は16本であった。結果として、ACPの対象者は、患者8件、患者と主介護者5件、主介護者2件、主介護者以外の家族員を含めた実践は1件であった。内容は、プログラム開発8件、ACP語の効果・検討６件、早期から実施可能とする要因1件、専門的プログラムを受けた訪問看護師と他職種によるACPの実践評価1件であった。ACPの実践の標準化や評価方法が検討されており、本研究の包括的家族カンファレンス作成に参考にできる。また形成期の社会的ハイリスク家族に、先駆的な地域での実践を提供しているフィールドにおいてインタビュー調査を（観察研究22207　子育て期家族への包括的家族カンファレンス実施可能性の検討）開始した。2023年1－3月、都内の子ども支援機関の代表者を介して、不登校の子どもを養育してきた経験のある養育者5名にインタビューを行った。家族構造、ファミリーヒストリー、家族ストレス対処行動、ファミリーネットワークの4構成概念に沿って、これまでの子育てについて振り返る包括的家族面接を実施した。結果として、ジェノグラムを記述する段階で、ファミリーヒストリーや家族のストレスに関する語りを引き出すことが可能となり、エコマップを描く段階では、ファミリーネットワークの活用や支援者との相互作用が肯定的変化をもたらしたことが語られた。
