不妊治療後の多胎妊婦が安心して出産・育児するための看護プログラムの開発と、その評価に関する研究を行った。前年度の研究結果、すなわち妊婦らがどのような不安を抱えているかの実態調査の結果を受けて、その不安を軽減するための看護プログラムとして視聴覚教材の開発を行った。視聴覚教材の内容としては「多胎新生児の出生時の様子」とその後の「多胎育児や成長の様子」が分かる冊子およびVTRを多胎消費者グループ・ネットワーク(2つの異なるグループ)の協力を得て作成した。本年度はその内容の検討を行い、さらにプレテストおよび、次年度以降の本格的な評価に向けての準備を実施した。プレテストの結果、当事者である多胎妊婦からは視聴覚教材を見ることで、"生まれた直後の子どもの様子(NICUでの子どもの状態、ケアを行う看護職者の様子、その場の雰囲気など)"を具体的にイメージできたり、育児中の生活場面では"大変さのなかでも親子で楽しそうに遊んだりする様子"などを見て、「少し安心できた」という声が多く聞かれた。また、臨床現揚で実際に多胎妊婦と接しケアをしている看護師ならびに助産師からは、視聴覚教材はとくに"育児中の現実的な生活状況"を知ることで、まずひとつは多胎育児に備えて妊婦やその家族が心身や生活環境を事前に整えることができること。次に、世間一般の、多胎の育児は単胎に比べると非常に大変で辛い、というだけのイメージから、楽しいこともたくさんあるのだということを妊婦が知って希望を抱けるようになる、といった意味での貢献度が大きい、との意見が聞かれた。以上、プレテストの結果から、本看護プログラムは多胎妊婦にとって育児中の不安を軽減し、安心をもたらすのに効果的であることが示唆された。上記を受けて、来年度以降は本プログラムをより多くの対象に実施し、その効果を評価してゆきたいと考える。
