緑膿菌は易感染性宿主の主要な病原菌であるが、近年薬剤耐性化傾向が高まっており、多剤耐性緑膿菌(MDRP)やメタロ-β-ラクタマーゼ産生緑膿菌(MBL-Pa)は高度先端医療を行う施設において脅威となっている。本研究では、多施設共同研究によりCase:MBL-Pa感染症とControl:MBL非産生MDR-Paを用いたCase-control studyにより臨床背景や予後の比較検討を行い、多剤耐性緑膿菌の中でも特に耐性度が高く予後も悪いMBL-Pa感染症と非産生MDR-Pa感染症の臨床像の相違を明らかにすることを目的とした。2施設合計52例(Case:MBL-Pa15例、Control:MBL非産生MDR-Pa37例)の患者群でCase-control studyを行った。検出検体では、Control群で気道系検体からの検出が多く、Case群では部位の特徴はみられなかった。抗菌薬使用歴では、カルバペネム系は有意差無く、キノロン系抗菌薬のみ有意差がみられた(53% vs 16%, P=0.11)。他のリスク因子では、尿道カテーテル(73% vs 49%, P=0.13)を含み、有意な因子は見いだせなかった。また、Case群においては、感染症が有意に死亡に関連していた(36% vs 0%, P=0.003)。以上より、MDRP感染者におけるMBL-Paのリスク因子として、キノロン系抗菌薬投与が有意の因子となり、予後も悪いため特に厳重な感染伝播防止策が必要であると考えられた。
