本研究は、生活スタイルを含めた環境改善による大学の精神的健康増進を行うために、アプローチすべき要因を実証的に解明することを目的としたものである。平成21年度は、健康診断のデータをもとに、学生の精神保健にとって一つの大きな問題である留年と関連する要因の解明、大学生でも問題となりつつある自殺の念慮や企図と関わる要因などについて解析を行った。まず学部2年生約3,000人を対象に、1年生から2年生に進学できなかった留年生の精神状態を解析したところ、留年生は非留年生に比べて、抑うつ・不安を示す質問紙(GHQ12)の得点が有意に悪く(p<0.01,OR=1.5)、幻聴様体験などの精神病様体験の頻度も高かった(p<0.005,OR=2.9)。また、入学時あるいはそれ以前の状態や既往と入学1年後の留年との関係をみると、入学以前に「自殺企図を考えたこと」のある学生では留年のリスクが高く(p=0.04,OR=2.6)、ほかに飲酒(p=0.04,OR=1.7)、「抑うつ」の既往(p=0.06,OR=2.0)などの影響が認められた。そこで「自殺念慮」や「自殺企図を考えたことがあること」がどのような要因と関連しているかを、入学後の健診データから検討した。その結果、性格における神経症傾向のほかに、「いじめられた体験」と精神病様体験とが、自殺念慮、自殺企図の考慮いずれとも有意な関連を示した(OR=2.7および3.1と、OR=2.7と2.8)。これらの結果から、1)大学の精神保健対策においては留年生のケアが一つの重要なポイントであること、2)大学入学以前からの「いじめ対策」、ならびに精神病様体験への注意とケアが、大学での精神保健対策を考える上でも極めて大切であることが示唆された。
