项目摘要

半随意的な運動及び音声チックを主症状とするトゥレット症候群(Tourette Syndrome)では症状に前駆衝動が伴う。そのためトゥレット症候群の当事者は、症状出現を事前に察知したり, 症状コントロールに向けて自己対処を行う場合がある。しかし、その対処内容や効果についての詳細は検討されていない。そのため、本研究ではチックへの自己対処の実態を把握し、自己対処に影響を与える文脈やチックの性質について理解した上で, 有効な自己対処について考察することを目的とした。本研究では自己対処の実態を把握する研究(研究1・2)、対処の有効性に影響する社会的文脈についての研究(研究3・4)、自己対処の前提となるチックの性質について検討する研究(研究5・6・7・8・9)の大きく3種の研究に分かれる。今年度前半は、チックの自己対処の行動実験に関する対象者を追加し、解析を進めた他、2本の論文を投稿し、うち1本が修正採択となり、再投稿後の審査中である。また、これまでの研究成果のうち、行動実験の部分と103名を対象とした自己対処の実態把握のための質問紙調査の部分を査読付きの国際学会(American Academy of Child and Adolescent Psychiatry)で発表した。今年度後半は3年間の研究を博士論文としてまとめ、東京大学大学院に提出し、審査の末、博士号を授与された。本研究は半構造化面接によって当事者の主観的な側面に焦点を当て、仮説及び臨床的な問いを設定した上で、質問紙研究、一事例行動実験や生理的指標等、幅広い研究手法を用いて、チックへの自己対処について深く検討しており、多角的な観点を用いた点が特徴である。チックの半随意性と患者の注意のあり方に焦点を当て症状の維持・悪化に関する認知・行動モデルを提示し、患者の自己対処をより有効なものにする臨床的示唆をもたらした点で特に意義が認められると考えられる。
