This subproject is one of many research subprojects utilizing the resources provided by a Center grant funded by NIH/NCRR. The subproject and investigator (PI) may have received primary funding from another NIH source, and thus could be represented in other CRISP entries. The institution listed is for the Center, which is not necessarily the institution for the investigator. This project is to conduct a randomized, double-blind, placebo controlled study comparing the effectiveness and clinical relevance of two forms of nicotine replacement (high versus low) with placebo. Data derived from this study will be analyzed in regard to a number of clinically and theoretically important questions, including the following: 1. Overall effectiveness of nicotine replacement treatment in this vulnerable group of patients, 2. Relative efficacy of the two types of treatment, 3. Relative efficacy of these treatment methods in patients who are also treated with different types of neuroleptics (atypical vs typicals), 4. Pharmacogenetics factors that might be associated with the success of smoking cessation. 5. The effect of smoking cessation on the expression of the enzymes 1A2 and the steady state of the neuroleptic and 6. The potential effect of smoke cessation on the side effect profiles, dosing and symptoms of patients.
