難病には様々な疾患があるが、日常生活を自らし、保健福祉制度の利用度の低い患者が災害時に孤立しやすいと考え、膠原病の患者・家族、患者会を対象に以下の3つの研究を実施した。研究1：実態を明らかにするため、全国膠原病友の会（患者会)の各支部役員にリモートによるインタビュー調査を実施した。患者会役員は、①対面での交流会が開けないこと。②リモート交流会の開催が難しいこと。③入会者の減少。④退会者の増加。⑤会員の状況が把握できないこと。⑥会費の徴収や減額。⑦役員の集会が難しいこと。⑧活動計画が立てられないことに悩み、困っていた。研究2：膠原病友の会A支部会員を対象に実施したアンケート調査を分析した。調査からは、半数以上の人が精神的な負担を抱え生活していた。熱や咳などの症状が出ると新型コロナの症状か膠原病の症状がわからず不安感じていた。研究3：全国膠原病友の会会員に郵送よる質問紙調査を実施した。新型コロナウイルス流行後も膠原病の病気は悪化していない人が多いものの、日常生活で精神的負担が増加した人、心に不調をきたしている可能性がある人が多いことが明らかになった。精神面に影響している要因として、膠原病の治療で免疫が低下しており感染したら重症化するのではないかという不安や、感染の機会を減らすために、外出を自粛し自宅にいる時間が増えていること、膠原病患が新型コロナウイルスに感染した場合のリスクや対応に関する情報が不足していること等が影響していると推察される。不安やストレスは、半数以上の人が、解消できていると回答していた。しかし、その一方で、不安やストレスを解消できていない人や精神科医療が必要な人も一定の割合でいることが明らかになった。以上の研究結果から、膠原病患者(難病患者)の感染症流行時の不安やストレスの軽減に関する取り組みや相談できる窓口の充実を図っていく必要があると考えに至った。

After the epidemic of new types of collagen disease, the disease will change, and the burden of daily life will increase. The possibility of heart disorder will increase. The main causes of mental illness are: low immunity, infection, severity, anxiety, decreased chance of infection, increased time spent away from home, insufficient information on new types of infection, collagen disease, etc. More than half of the people in the country are worried about the situation. A person's mental health care needs to be cut off. The results of the above study indicate that there is a need to investigate the prevalence of infectious diseases in collagen patients (refractory patients).
