これまで私たちは胃癌組織の分化と遺伝子異常の関わりを報告してきた。胃癌発生初期の形質をよく保持していると考えられる粘膜内癌を対象として、癌細胞の細胞分化を、胃と腸の粘液に特異的な抗体を用いて調べ、同時にこれまで大腸癌や胃の分化型腺癌(intestinal type carcinoma)の発生に関与しているとされる5q上でAPC遺伝子に近接しているmicrosatelliteの変化を検索した。検索した胃の粘膜内癌34例のうち16例(47%)に5q上でAPCに近接したmicrosatelliteの異常(loss of heterozygosity 6例、microsatellite instability10例)を認めた。組織型・細胞分化をみると、分化型癌24例のうち11例で胃型形質が優位で残りの13例では腸型形質が優位であった。未分化型癌10例中7例で胃型形質が優位であった。LOHが確認できた6例は全て腸型形質のみを発現する癌(分化型5例、未分化型1例)であった。この領域の遺伝子のdeletionは胃癌のなかでも腸上皮化生と関連して発生するような腸型の分化型腺癌の発生に関与しており、胃型腺癌の多くの未分化型癌の発生との関連性は乏しと思われた。

This is a report on the differentiation of gastric cancer tissue and the abnormality of the fetus. The early stage of gastric cancer is characterized by the preservation of its morphology and the detection of intramucosal cancer, cell differentiation of cancer cells, gastric and intestinal mucus-specific antibodies, and the detection of microsatellite transformation in colorectal cancer and gastric differentiated adenocarcinoma. 16 cases (47%) of 34 cases of gastric intramucosal carcinoma were found, and the APC near 5q was abnormal (loss of heterozygosity 6 cases, microsatellite instability10 cases). Histological type: cell differentiation: 24 cases; gastric type: 11 cases; intestinal type: 13 cases; 7 of 10 cases of undifferentiated carcinoma were gastric type. LOH was confirmed in 6 cases, but not in all cases (differentiated in 5 cases and undifferentiated in 1 case). The deletion of this domain is related to the development of gastric cancer, intestinal metaplasia, and gastric adenocarcinoma, and to the development of undifferentiated carcinoma.
