本研究では、診療決断に迫られたとき、患者の意思を考慮し意思決定おこなうための支援モデルの基礎について考察を行った。転帰に対する患者の持つ価値観に基づく意思決定問題としては,木構造を持つ問題が一般的に用いられる。これは、フォンノイマン型効用理論を応用した意思決定問題として提案されている.この問題における期待効用の導出に対しては,ユニークな確率測度上で議論がなされている。しかし、発生する転帰に異なる事象確率が報告されることは珍しくなく、いずれを選択すべきが迷うことがある。また、患者の意思を転帰においてではなく,転帰にいたる過程上でその不確実性を表現できるものではなかった.前述の問題点を考慮し,患者の意思が複数の確率測度から構成され,最も悲観的な状況下においても,より良い結果を得られる判断を下す意思決定問題の定式化について考察を行なった.つまり,不確実性回避的な患者の意思を考慮する意思決定問題となっている.現在、患者の意志を表現する生活の質に対し,質のみならず,量的な要素も考慮すべく,さらに,時間の推移とともに変動する生活の質を考慮可能な新しい手法として注目を浴びているQ-TWiSTに対しても,複数の確率測度を認める場合の定式化について提案を行った.患者の意思を不確実性回避的であると仮定しているが,リスクを負いながらも良い転帰を向かえるべく積極的に不確実性を受け入れる患者へのモデル作成も必要であると考えられる.
