STRATEGIES FOR THE TREATMENT OF HIV ASSOCIATED METABOLIC SYNDROME

This subproject is one of many research subprojects utilizing the resources provided by a Center grant funded by NIH/NCRR. HIV-associated metabolic syndrome, characterized by body fat redistribution, dyslipidemia, and insulin resistance, is frequently observed in patients treated with highly active antiretroviral therapy (HAART). The underlying mechanisms are still unknown. However, the presence of metabolic abnormalities, including dyslipidemia and visceral adiposity, suggests an increased risk for developing cardiovascular disease in HIV-infected patients, especially given the prolonged duration of survival associated with HAART. Presently, there are no established treatment options for this syndrome. However, because diet and exercise has improved the metabolic profile of HIV negative individuals, it is important to explore the potential beneficial effects of such strategies in HIV-positive patients. In this study we will focus on determining the optimal program for HIV-infected patients with the metabolic syndrome by evaluating lifestyle modification in combination with insulin sensitizing agents (metformin) and progressive resistance training.
