Conditions of pulmonary preservation have been assessed by the alveolar fluid clearance with the fluid-filled rat lung model and following results have been obtained sofar.(1) 24-hour preservation with EP4 solution showed significantly superior result compared with that with Euro-Cllins solution.(2) In spite of good result, the functional enhancement of Na-K-ATPase by Terbutaline was demonstrated to be damaged even if the lung was preserved for 24 hours with EP4 solution.(3) Dexamethazone proved to enhance the Na-K-ATPase function 72 hours after administration, which demonstrated the beneficial effect of steroid hormone in pulmonary preservation.(4) Using cultured alveolar epithelial cells, IL8 production after the exposure of the cold preservation found to be lower in EP4 solution than in Euro-Collins solution, which was improved by the supplement of Dexamethazone in the cultured solution.These results indicate the superiority of EP4 solution in pulmonary preservation which has been suggested by canine or primate lung transplantation models. Also, steroid treatment for the reperfusion injury after lung transplantation proved to be appropriate from the result of IL8 assessment by cultured alveolar epithelial cell. Based on these basic results, further improvement of pulmonary preservation can be expected.
