SEARCH FOR DIABETES IN YOUTH OF THE PUGET SOUND

This subproject is one of many research subprojects utilizing the resources provided by a Center grant funded by NIH/NCRR. Data on prevalence and incidence of diabetes in US youth is limited and proportions with various diabetes types are unknown. Clinical factors and insulin requirements no longer reliably differentiate types of childhood diabetes. SEARCH for Diabetes in Youth is a national, multi-center, uniform approach to case ascertainment, typology, surveillance and research on diabetes in children and adolescents that will measure the extend of diabetes in the US population of youth under the age of twenty by type of diabetes, age, gender and race / ethnicity. Secondary objectives include developing practical approaches to identifying diabetes type, describing childhood diabetes and its complications by type, and characterizing health care utilization, processes of care and quality of life. Researchers at six centers across the country are conducing surveillance on 6 million youth with wide ethnic and socioeconomic representation (~6% of the 20 UW population), identifying approximately 6000 prevalent diabetes cases in 2001, and 800 incident cases per year in 2002-2004. The Washington study population is all youth with diabetes in 2001-2004 who are under age 20 and reside in the Puget Sound, which includes approximately 1500 prevalent and 600 incident cases. SEARCH is collecting information through limited data, surveys, brief physical examinations (height, weight, blood pressure, waist to hip measurement and assessment for acanthosis nigricans), laboratory studies (diabetes autoantibodies, fasting and stimulated C-peptide, hemoglobin A1c, blood glucose, lipids, urine albumin, creatinine, DNA extraction, and plasma storage), and interview surveys (date and setting of diagnosis; clinical presentation, age and body size at diagnosis; other medical history and use of prescription medications; race / ethnicity and socioeconomic status, family structure, and quality of life).
