The present study built a safety management approach particularly from the following two aspects, aiming at mitigating accident risk in healthcare : (1) fostering a positive safety culture within an organization and (2) introducing patient views to risk management. Firstly, we yielded important characteristics on healthcare safety culture in Japan as well as developed risk management techniques and tools based on its measurement and diagnosis. Secondly, as an introductory study of patient centered risk management, we uncovered patient wishes, views and perceptions to healthcare organization and staff when they had been victims of adverse events. Based on the results obtained throughout this project, we provided several suggestions for improving patient safety in Japan.

本研究建立了一个安全管理方法，特别是从以下两个方面，旨在减轻医疗事故风险：（1）在组织内培养积极的安全文化和（2）引入患者的意见，风险管理。首先，我们得出了日本医疗安全文化的重要特征，并在其测量和诊断的基础上开发了风险管理技术和工具。其次，作为以患者为中心的风险管理的介绍性研究，我们揭示了患者的愿望，意见和看法，医疗机构和工作人员时，他们已经成为不良事件的受害者。根据整个项目的结果，我们为改善日本的患者安全提供了几项建议。
