Developing Risk Management Systems for Patient Safety : Introduction of Patient Views and Fostering Effective Safety and Learning Culture for Healthcare Management

本研究特别从以下两个方面建立安全管理方法，旨在降低医疗保健中的事故风险：(1)在组织内培养积极的安全文化；(2)将患者的观点引入风险管理。首先，我们得出了日本医疗安全文化的重要特征，并基于其测量和诊断开发了风险管理技术和工具。其次，作为以患者为中心的风险管理的介绍性研究，我们揭示了当患者成为不良事件的受害者时，他们对医疗保健组织和工作人员的愿望、观点和看法。根据整个项目获得的结果，我们为提高日本的患者安全提供了几点建议。

The present study built a safety management approach particularly from the following two aspects, aiming at mitigating accident risk in healthcare : (1) fostering a positive safety culture within an organization and (2) introducing patient views to risk management. Firstly, we yielded important characteristics on healthcare safety culture in Japan as well as developed risk management techniques and tools based on its measurement and diagnosis. Secondly, as an introductory study of patient centered risk management, we uncovered patient wishes, views and perceptions to healthcare organization and staff when they had been victims of adverse events. Based on the results obtained throughout this project, we provided several suggestions for improving patient safety in Japan.
